Our CHWs work directly with members, helping them navigate their healthcare journey and connecting them with vital services that improve their health outcomes.
Here are three recent success stories that demonstrate the transformative impact of Mobile Integrated Health and our CHWs:
Supporting an Elderly Member in Transition
An elderly member was dissatisfied with her current incontinence and C-Pap supply store, which she had relied on for years. Understanding her frustration, one of our CHWs stepped in to provide referrals to other pharmacies within her insurance network. These referrals ensured the member could receive her much-needed supplies while being treated with respect and dignity.
The CHW also assisted the member in setting up an online account, which she used to complete her health risk screener. By calling member services together, they secured a gift card for the completed screener—a meaningful benefit for someone on a fixed income. With continued education on value-added benefits like transportation and rewards programs, the CHW has become a trusted resource for the member, empowering her to better manage her healthcare needs.
Additionally, the CHW helped the member with recertifying for all state programs, including Medicaid, SNAP, and LIHEAP. As the member continues navigating life on a fixed income, the CHW guided her through applying for rental assistance while she awaits the review of her Section 8 application, which will help make her rent more affordable.
Navigating Medicaid for a Spanish-Speaking Mom
A young Spanish-speaking mother was struggling to secure Medicaid coverage for her child. After being misinformed by a local clinic that her child’s coverage was active, she sought help from one of our CHWs. Working alongside an interpreter, the CHW clarified the situation and assisted the mother in reapplying for Medicaid.
Recognizing the mother’s broader challenges, including food insecurity and a lack of baby supplies, the CHW provided information on local food pantries and baby boutiques. These resources offered immediate relief while the CHW continued to provide follow-up support to ensure the Medicaid application was processed correctly. Thanks to the CHW’s efforts, the mother now feels more seen, supported, and connected to the resources she needs.
Rebuilding a Life with Compassionate Support
Another member, a mother grappling with addiction, legal challenges, and unstable housing, faced issues with her Medicaid coverage. When her pharmacy denied a prescription refill due to inactive coverage, a CHW stepped in. Meeting the member at a local library, the CHW helped her contact the Department of Health and Human Services to update her case, enabling her to access her medication.
The CHW also assisted the member in completing her health risk screener and provided guidance on finding affordable housing—a crucial step as she works to regain custody of her children. With this ongoing support, the member is taking meaningful strides toward stability and better health.
