How MTM’s Mobile Integrated Health Program Empowers Medicaid Members in Need
Accessing healthcare and support services in rural areas can be a significant challenge, especially for Medicaid members who may already be struggling with other issues. MTM’s Mobile Integrated Health (MIH) program is designed to bridge this gap, offering personalized support to help members navigate these challenges. A recent example of this impact comes from one of our Community Health Workers, Bonn A., who worked closely with a member facing multiple crises.
When Bonn first reached out, the member was hesitant, often saying it “wasn’t a good time.” However, with patience and persistence, the member eventually opened up, revealing they were in urgent need of rental and utility assistance. Bonn immediately stepped in, meeting with the member to review a 30-day vacate notice and helping them explore assistance resources. Together, they applied for the support that best suited the member’s needs.
During their conversations, the member also mentioned suffering from dental pain due to untreated dental issues from over a year ago. Bonn quickly connected them with a dental clinic for emergency treatment and set up ongoing dental care with another provider.
A few days later, the member reached out to Bonn late in the evening, seeking behavioral health resource information. Bonn responded swiftly, providing the necessary resources to help the member through their personal crises, including food assistance after severe weather further impacted their situation.
Over a two-week period, Bonn had 15 encounters with the member, working closely with them to navigate resources, access care, and find personal assistance. This story highlights how MTM’s MIH program makes a tangible difference in the lives of Medicaid members in rural areas, offering critical support and building trust, one step at a time.
