Maternal and infant health outcomes in the United States continue to be challenged by limited access to care, health disparities, and social determinants of health. More than 2.3 million women of reproductive age live in maternity care deserts, with millions more facing restricted access to prenatal and postpartum care. These gaps contribute to high rates of adverse pregnancy outcomes, even as the U.S. spends more per capita on maternal healthcare than any other nation.

At MTM Health, we are working to close these gaps through our Mobile Integrated Health (MIH) solutions, which bring care directly to individuals in need—including mothers and infants—in their homes and communities. Our Community Health Workers (CHWs) play a pivotal role in providing education, connecting members to resources, and advocating for better outcomes. Through these efforts, we are improving maternal and infant health outcomes one family at a time.

Real-Life Stories: Transforming Lives through MIH

Empowering New Mothers Through Education and Support

One of our CHWs recently worked with a young mother who had struggled to obtain a breast pump. After attempting to order one through an online vendor, she was wrongly informed that her insurance plan would not cover it. Frustrated, she considered changing her health plan. During a follow-up postpartum appointment, she shared her concerns with our CHW, who stepped in to help.

By educating the mother about her current health plan’s benefits and facilitating a conference call with member services, the CHW helped her discover that not only was she eligible for a breast pump, but she also qualified for additional benefits—including a car seat for her baby and gift cards for preventive visits. What started as a frustrating experience turned into a moment of relief and empowerment, all thanks to the personalized support of our CHW.

Supporting a Single Mother in Crisis

Another success story highlights how MIH can be a lifeline for struggling families. A single mother of four, who had recently left a domestic violence situation, was facing financial instability and mounting stress. After completing a health risk screener, our CHW identified several areas where she needed urgent support.

Through our MIH program, the CHW helped the mother navigate financial relief options, such as recertifying her rental assistance and exploring income-based housing. She also assisted in securing much-needed infant formula through the state’s Medicaid program when pharmacies were unable to fill the prescription. Additionally, the CHW arranged holiday gifts for her children through a local community program, alleviating some of the burdens of the season. With these crucial interventions, the mother now has a clearer path forward and is working toward her goal of becoming a nurse.
