Mobile Integrated Health Solutions in Action: A Member Success Story

Accessing healthcare doesn’t always have to begin and end with a ride. At MTM, we believe members deserve innovative solutions that go beyond transportation—a goal we achieve by focusing on the healthcare event and the member experience. Designed to revolutionize the way health plans nationwide deliver healthcare—particularly in rural communities—our Mobile Integrated Health (MIH) model represents a comprehensive approach to bringing care directly to members, wherever they may be. Here’s a recent example of our MIH solutions in action and delivering positive outcomes!

In late June, one of MTM’s health plan partners referred a 62-year-old member for an urgent home safety visit and re-engagement with health services. Living in a rural, underserved county without a phone, the member faced significant barriers to accessing care. Julie S., the nearest Community Health Worker (CHW) with MTM, promptly responded to the referral.

Upon receiving the referral, Julie immediately drove to the member’s residence to conduct a home visit. She observed cleanliness issues and excessive clutter, which hindered the member’s ability to move safely using her walker. Julie discussed these concerns with the member’s caregiver, offering suggestions to improve home safety. During the visit, the member also requested assistance with personal care, including bathing and toileting, as well as help with scheduling dental and eye care appointments.

Over the next two weeks, Julie made multiple visits to address critical social determinants of health (SDOH) and enhance the member’s living conditions. She registered the member on the health plan’s portal, reviewed available Medicaid resources, and verified the removal of clutter to ensure safe mobility. Julie also guided the member through a Health Risk Screener, helped her brother find a part-time job, and connected the member with caregiving services for daily living assistance. Additionally, Julie coordinated with a local hospital social worker to align care plans and prevent service gaps, recertified the member for her SNAP benefits, and arranged phone and internet services through the Housing Authority.

The member expressed excitement and relief at having Julie’s support, recognizing the significant improvement in her quality of life. The health plan is equally pleased to have regular contact with the member, ensuring she receives the necessary resources and assistance. Julie’s dedication and immediate action highlight the vital role of CHWs in supporting vulnerable populations and addressing social determinants of health.
