It is imperative that providers report all accidents and incidents to MTM, regardless of severity. In addition, if a vehicle is involved in an accident, the driver should immediately call the police to obtain a police report. Sometimes members may not seem hurt at the time of the accident; however at a later time will report that they sustained injuries. A police report will help to protect your company and MTM in these situations.

Please utilize the following Accident and Incident Forms to report occurrences:

Accident and Incident Form - Nevada: This form must be submitted to MTM by the end of the business day following the accident or incident; injury reports must be made verbally to MTM within three hours of the occurrence.

Accident and Incident Form - All Other Locations: This form must be submitted to MTM by the end of the business day following the accident or incident; injury reports must be made verbally to MTM within three hours of the occurrence.
